4. Legal basis and consent
We only email you if you've affirmatively checked a consent box and provided your email. You can withdraw consent at any time by using the unsubscribe link in any email or by contacting us directly (see Section 9).
5. Health information — an important note
This checklist is for self-reflection and awareness only. It is not a medical device, does not provide a diagnosis, and your results are not reviewed by a medical professional. Because we are not a healthcare provider, this checklist and the data you share here are not covered by HIPAA. We still treat your information carefully: we store only your summary result (never your individual symptom answers) precisely to limit the sensitivity of what's tied to your email.
6. Where your data is stored and who processes it
- Cloudflare hosts this site and stores your submitted information (email, optional age range, result summary) in a Cloudflare D1 database.
- If you opt in to a follow-on email series, your email and result summary are shared with our email service provider [ESP NAME — TO BE CONFIRMED once selected] solely to send you that series. That provider is contractually restricted from using your data for any other purpose.
- We do not use any other third-party advertising or data-broker services on this page.
7. How long we keep your data
We retain your information for as long as you remain subscribed to receive emails from us, plus a reasonable period afterward for record-keeping. If you unsubscribe or request deletion, we will remove your personal information within 30 days, except where we're required to retain limited records by law.
